I'd add another thought. Add "huddles" to your routine. Huddles are short (think 10 minutes max) mini-meetings. Start by having each team member share good news. It can be business or personal. Next have them share anything that is "on their plate." Not everyone will have something to share, but some will. Finally, ask if anyone is stuck or needs help with something. This will often bubble up an issue at the early stages when it can be addressed before it blows up into a crisis. These don't need to be daily huddles. two to three times a week will suffice.
